USA Hockey Code of Conduct - REQUIRED to be completed for all players, coaches and managers

USA Hockey Consent to Treat -  REQUIRED to be completed for all players and coaches

Player Commitment Form - REQUIRED to be completed for all players

Waiver of Liability Form - Required to be completed for all players, coaches and Managers

Coaching Ethics Code of Conduct - Required to be completed by all coaches

Student Coach Information Form- Required for any student coach

New Travel Jersey Order Form - Required for any new travel jersey orders

Travel Jersey Exchange Day - Flyer for Travel Jersey Exchange Day

Player Injury Report  (PDF)  (Word)- This document needs to be filled out any time an injury requires medical attention.

USA Hockey Player Awards - Use this document to track player accomplishments during the year: hat tricks, play makers, shutouts.

Sample Team Roster - Including parents names, address, phone, etc.

Player Check List - This is a form to use as a check list when collecting forms from players - such as birth certificate, medical release, USA hockey card etc;

Hotel Request Form - A sample form for parents to fill out so managers can reserve hotel rooms for tournaments.
